Description IRIS Consultant Full-Time | Hybrid Work Arrangement Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a compassionate, organized, and self-motivated IRIS Consultant to join our IRIS (Include, Respect, I Self-Direct) team. In this hybrid role, you will support older adults and individuals with disabilities who self-direct their long-term care services, helping them maintain independence, achieve personal goals, and live meaningful lives within their communities. As an IRIS Consultant, you will build trusting relationships with participants, assist with developing and monitoring individualized support plans, connect people to community resources, and ensure health and safety needs are met. This position combines remote work, community visits, and collaboration with team members across northern Wisconsin. What You'll Do
