POSITION SUMMARY

The Addiction Treatment Program Counselor provides counseling, education, and clinical support to patients served within the program. This role is responsible for delivering individualized therapeutic services, supporting treatment planning, and assisting patients in achieving recovery goals through evidence-based interventions, care coordination, and discharge planning. The counselor also collaborates with internal and external partners to ensure continuity of care and effective utilization of services.


ESSENTIAL FUNCTIONS

  • Develop and implement individualized treatment plans based on patient needs and clinical assessments
  • Complete counseling assessments to evaluate patient needs, strengths, and treatment goals
  • Obtain pre-authorizations and continued authorizations for treatment services
  • Refer patients to appropriate agencies and community resources to address behavioral health, substance use, and social service needs
  • Review and update treatment plans regularly to reflect patient progress and clinical changes
  • Facilitate educational lectures, therapeutic groups, and recovery-focused programming
  • Provide individual counseling and therapeutic support to patients in the program
  • Develop comprehensive aftercare and discharge plans to support long-term recovery and continuity of care
  • Collect and maintain statistical data to support program evaluation and effectiveness monitoring
  • Perform other related duties as assigned


QUALIFICATIONS
Education

  • Master's degree in Social Work required


Licensure / Certification / Training

  • LMSW licensure required


Experience

  • Minimum of one year of clinical experience in a hospital, mental health, or substance abuse treatment setting


Knowledge, Skills, and Abilities

  • Strong customer service, communication, and organizational skills
  • Ability to communicate effectively in person, by phone, and through documentation and electronic systems
  • Ability to work effectively in a fast-paced, high-stress environment
  • Strong reading comprehension, mathematical reasoning, adaptability, and problem-solving skills


Salary Range: Minimum 35.00/hour - Maximum 57.71/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
