Summary:

Breast Care Patient Navigator Nurse serves as a main point of contact for patients, caregivers, and physicians to provide resources and assistance with accessing clinical and supportive care services offered within the healthcare system and in the community. The Nurse Navigator is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care, assisting the patient throughout their diagnosis/treatment, and aiding in overcoming any barriers to care. Facilitates appointments for consults and support services within established standards. Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services. Secures and executes financial grants from state and/or national programs if applicable and assures compliance. Works with Cancer Registry staff and others to collect data, track outcomes, and support. Helps with educational programs for patients and within the community regarding breast care and health.

  • Appropriately adapt assigned patient assessment, treatment, and/or care methods to accommodate the unique physical, psychosocial, cultural, spiritual, age-specific, and other developmental needs of each patient served.
  • Ensures compliance with all policies, procedures, and standards of care as deemed appropriate by State and Federal agencies, the hospital, and other regulatory entities.
  • Demonstrates flexibility by working other shifts and at other campuses as needed or required.
  • Performs other duties as assigned.

Job Requirements:

Education/Skills

  • Bachelor's degree in Nursing preferred

Experience

  • 3 years of nursing experience required
  • Experience in oncology or case management or nursing specialty preferred
  • Basic computer experience required

Licenses, Registrations, or Certifications

  • RN or LVN/LPN license required
  • BLS required

Work Schedule:

8AM - 5PM Monday-Friday
