This position is perfect for an RN who thrives on autonomy and clinical coordination. You will serve as the primary link between patients, families, and elite physician teams, ensuring that patients with chronic kidney disease (CKD/ESRD) receive seamless, high-quality care.

• Experience: Minimum of 3 years in clinical nursing (Required).

• Leadership: Background in healthcare performance coaching or acting as a "change agent."

• Preferred: Renal nursing experience, BSN, or certifications like CNN or CCM.

• Logistics: Valid CA driver's license with a positive record.
