POSITION SUMMARY:

The Ventricular Assist Device (VAD) Coordinator reports to the Cardiac Surgery Service Line Director of the Cardiovascular Institute with collaboration with the surgical and medical director of the VAD program. Primarily responsible for the planning, direction, and operation of the administrative and clinical aspects of the patient coordination component of the VAD Program. Directs and coordinates resources for the delivery of care to patients and their families. In collaboration with the chief of cardiac surgery and heart failure director physician leadership provides direction impacting patient care including but not limited to, screening, education, and coordination of care from the initial patient visit through VAD management.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

Coordinate the preoperative evaluation and postoperative management of VAD patients.

Act as the primary point of contact for VAD patients and their families, providing education, support, and resources.

Develop and implement individualized care plans in collaboration with the healthcare team.

Monitor patients' progress and respond to changes in their condition.

Work closely with cardiologists, surgeons, advanced practice providers, nurses, and other healthcare professionals to ensure seamless patient care.

Coordinate VAD management across pre-, intra-, and post- implant.

Facilitate multidisciplinary team meetings and case conferences.

Communicate effectively with all team members to coordinate care and optimize patient outcomes.

Provide comprehensive education to patients, families, and caregivers about VAD management, including device operation, troubleshooting, and lifestyle modifications.

Develop and deliver training programs for healthcare staff on VAD protocols and best practices.

Create and distribute educational materials and resources.

Program Development and Quality Improvement:

Assist in the development and implementation of VAD program protocols and policies.

Participate in quality improvement initiatives to enhance patient care and program efficiency.

Collect and analyze data related to VAD outcomes and program performance.

Ensure compliance with all regulatory standards and guidelines related to VAD management.

Maintain accurate and up-to-date patient records, including documentation of care plans, patient interactions, and clinical outcomes.

Prepare and submit reports to regulatory bodies as required.

Advocate for the needs and preferences of VAD patients within the healthcare system.

Provide emotional support and counseling to patients and families facing the challenges of living with a VAD.

Connect patients and families with community resources and support groups.

Performs other duties as assigned.

BASIC KNOWLEDGE:

Associates or Baccalaureate Degree in Nursing or Master's degree from an accredited Nurse Practitioner program.or graduate of a Physician Assistant Program with an M.S. degree required.

EXPERIENCE:

Experience in cardiac care or critical care is required.

Experience as a VAD Implant coordinator strongly preferred

Excellent written, verbal and interpersonal communication skills.

Experience and skills with electronic health record required.

Knowledge of and compliance with RIDOH, Joint Commission, DNV, HIPPA, ACGME and other regulatory bodies and standards.

Computer skills including Microsoft Office.

Pay Range:

$122,387.20-$155,292.80
