Hourly Pay Range:

$21.08 - $31.62 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors.

Position Highlights:

  • Position: Patient Navigator

  • Location: Corp Center ; Warrenville, IL

  • Part Time: Full Time

  • Hours: 9:00am-5:30pm

  • Required Travel: No

A Brief Overview:
The Navigator will assist patients with getting access to social services and other community-based resources as needed. They will be responsible for establishing trusting relationships with patients and their families while providing culturally appropriate support and encouragement. Additionally, the Navigator provides administrative support, including faxing, scanning, scheduling filing and general organization.

What you will do:

  • Assist patients with access to social services and community-based resources; document all patient interactions

  • Assist patients with scheduling and appropriate referrals; this includes answering telephone calls, returning messages, scheduling appointments, communicating with other hospital departments to manage and obtain referrals.

  • Maintain inventory of office and medical supplies

  • Assist with grant functions as needed; this may include data entry and maintenance, and patient interaction.

What you will need:

  • High school education or equivalent

  • 1-yr clerical work experience
