Valley Medical Group, part of the Valley Health System in Paramus, New Jersey, is seeking a full-time Geriatrician to join our growing Medical Specialties Service Line. The Geriatrician will maintain a clinical practice, providing comprehensive clinical care to older adults, including consultations, assessments, primary care, and follow-up visits. Responsibilities include detailed geriatric evaluations focused on overall health, functional status, cognitive function, support systems and the identification of cognitive impairment, dementia, and related neurocognitive conditions. The physician will manage both acute and chronic medical conditions with attention to key geriatric syndromes such as dementia, falls, polypharmacy, delirium and frailty. The role involves active participation in care plan meetings and family conferences to discuss treatment options and advanced care planning. The physician will also work collaboratively with the multidisciplinary team in skilled nursing facility to provide clinical management, smooth transitions of care and reduce avoidable readmissions.
