Summary
Responsible for assisting patients and their families in navigating housing options related to healthcare needs. This role involves coordinating housing resources, providing support to patients facing housing challenges, and collaborating with community organizations to ensure patients have access to safe and affordable housing during and after their medical treatment.

Essential Functions
-Assess patients' housing needs and provide guidance on available resources and options.
-Coordinate with social services, community organizations, and housing authorities to connect patients with appropriate housing assistance programs.
-Develop and maintain relationships with local housing providers and organizations to enhance available resources for patients.
-Facilitate communication between patients, families, and healthcare providers to ensure housing needs are addressed as part of the overall care plan.
-Assist patients in completing applications for housing assistance, subsidies, or other relevant programs.

Education
High School Diploma or Equivalent required or Associate's Degree Social Work preferred

Experience
Experience in social services, case management, or a related role 2-3 years required and Experience within a healthcare or community service environment 1-2 years preferred

- Strong understanding of housing resources, assistance programs, and community services.
- Excellent interpersonal and communication skills for engaging with patients, families, and community partners.
- Ability to assess individual needs and develop tailored support plans.
- Strong organizational skills to manage multiple cases and maintain accurate records.
- Empathy and sensitivity to the challenges faced by patients regarding housing stability.
- Problem-solving skills to address barriers and find solutions for patients in need of housing.
