Duration: 12+ Months

Hours/Week: 15 Hybrid (Some travel required; including quarterly in person meetings)

Position Summary:

  • We are seeking a dedicated Outreach Specialist to join our pharmacy team and play a critical role in improving access to care for Medicaid beneficiaries. The Outreach Specialist will provide oversight of access barriers and act as lead liaison to Non-Emergency Medical Transportation (NEMT) services. This role involves supporting Community-Based Organizations (CBOs) by developing training programs on access barriers, NEMT processes, and best practices for screening, referrals, and beneficiary education.
  • The Outreach Specialist will also oversee the delivery of case management and care coordination services by CBOs and ensure accurate and timely payment for services rendered. Role & Responsibilities:

  • Provide strategic oversight of access barriers and NEMT services.
  • Develop and deliver training for CBOs on access barriers, NEMT procedures, and beneficiary engagement.
  • Monitor and support CBOs in providing screening, referrals, and education to beneficiaries.
  • Oversee case management and care coordination efforts to ensure high-quality service delivery.
  • Ensure appropriate documentation and payment processing for services provided by CBOs.
  • Collaborate with healthcare providers, transportation vendors, and other stakeholders to reduce treatment access barriers.
  • Maintain accurate records and generate reports as needed to inform program evaluation and improvement.
  • Stay informed on state and federal guidelines regarding NEMT and care coordination services.
  • Other related tasks as assigned.


Required Qualifications:

  • Bachelor's degree in a relevant field, such as public health, health education, nursing or social work
  • Demonstrated experience working with individuals living with rare hematologic conditions.
  • Strong understanding of healthcare access barriers and transportation challenges faced by Medicaid beneficiaries.
  • Proven ability to develop and deliver training programs to community partners or staff.
  • Excellent communication, organizational, and case management skills.
  • Ability to build and maintain relationships with CBOs, healthcare providers, and clients.
  • Experience in oversight or administration of service payments.
  • Cultural competency and sensitivity when working with diverse populations.
  • Comfortable traveling to CBOs throughout Rhode Island as needed.


Preferred Skills:

  • Prior experience with Medicaid or public health programs.
  • Knowledge of local community resources and support networks.
  • Possess a valid and active license as a Licensed Independent Clinical Social Worker (LICSW), Licensed Social Worker (SW), Registered Nurse (RN), or Licensed Practical Nurse (LPN)
  • Certified Health Education Specialist (CHES), Master Certified Health Education Specialist (MCHES), or Certified in Public Health (CPH)
