Healthy Communities Program Manager

The Healthy Communities Program Manager is responsible for planning and implementing events and programs to promote community health. This position collaborates with internal and external partners and is responsible for program development, implementation and management. The position works as part of the Center for Health Equity to achieve improved health outcomes through project coordination, stakeholder engagement, financial management, and reporting.


Leads the development of community health initiatives and collaborates with local partners to assess gaps in access and implement diverse programming promoting healthy eating, active living, mental health, social health, and addressing social drivers of health to improve community health outcomes.
Works with local community leaders and coalitions to assess health service access needs for specific communities and populations, developing action plans to address gaps and leading the implementation of programming and events to enhance support for specific populations.
Strengthens community relationships by representing Cone Health on various community teams and collaboratives, fostering partnerships to expand healthcare initiatives and community benefit efforts.
Develops the overall structure and timeframes for community health programming and collaborates with internal and external partners to facilitate a structured collaborative improvement process, using tools of continuous improvement and program evaluation to assess and enhance the impact of initiatives and programs aimed at improving community health outcomes.
Performs other duties as assigned.

EDUCATION:
Required: Bachelor's Degree

EXPERIENCE:
Required: 5 Years
