Review and reconcile physicians' orders and update medical data. Communicate pertinent information to involved parties according to system guidelines. Primary contact will be with clinicians, physician practices, acute care facilities, skilled nursing, extended care facilities, medical supply companies as well as outside agencies for both current and potential patients. Ensure that physician orders and all Medicare-required documentation are received and communicated to the appropriate staff. Maintain professional and collaborative internal and external relations with contacts.
Does this position require Patient Care? No Essential Functions: -Ensure that clinical and non-clinical data is updated and corrected in the electronic medical record (EPIC) as appropriate. -Generate inter-agency discipline referrals based on the clinician's request. -Assists with patient-related issues resulting in calls to doctor's offices as well as other health care providers as directed by case managers. -Assists and may coordinate follow-up with various labs and obtain clinical orders per physician's request. -Ensure that clinical documentation and medical orders are entered in accordance with agency standards. -Coordinate and/or assist with ordering medical supplies and DME and ensure proper follow-up when needed. -Assist with patient assessments, including vital signs, and report changes in patient condition to the care team.
