Canvas Health's mental health and substance use disorder providers, clinicians, and staff bring their diverse lived experience to help inform and guide the care they provide to clients. Our integrated programs and services offer coordinated care to people of all ages in their homes, the community, and in our clinics. We strive for excellence in clinical care as we serve those living with mental illness, substance use disorders, crisis, unstable housing, and trauma. Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary The ARMHS staff provide adult rehabilitative mental health services that instruct, assist, and support the recipient in basic living and social skills to individuals with serious mental illnesses or serious and persistent mental illnesses. These activities are to help restore a recipient's skills for managing their illness and everyday independent living. The service areas would include: interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, medication education and monitoring, mental illness symptom management skills, household management skills, employment-related skills, and transition to community living services. It is collaborative in nature and requires working closely with Community Services, Court Services, private providers, outside resources, and programs at Canvas Health. Requires the ability to drive to individuals' homes or in the community when needed. This position will primarily be serving individuals in Ramsey and Washington counties. Location Hybrid Schedule 40 hours, Monday through Friday, 8am-5pm, meeting with clients in their homes or communities or over telehealth or both.
