The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. Job Overview: The Community Case Manager will play a vital role in supporting individuals within the community by initiating and coordinating medical care and services for underserved and marginalized populations through increased health literacy and improved access. This role is essential for improving patient outcomes and ensuring individuals receive comprehensive and effective care. The CCM health professional will focus on disease prevention and delivering education and resources that allow a community population to improve general wellness through sustainable behavior change. Qualification Required:
