Job Description:

In addition to the Compensation Details above, the incumbent in this position will be eligible to receive location pay ($4,000/year).

Responsibilities

The Health Program Coordinator will be responsible for engaging with clients referred from community partners, including, but not limited to the New York Police Department (NYPD), and other law enforcement agencies. The incumbent will conduct outreach and aid in providing client navigation and linkage to services. Responsibilities include conducting home visits, making phone calls, and performing follow up with clients within New York City, offering harm reduction and overdose prevention coordination of care, providing tailored prevention messages to participants based on risk behaviors, and providing referrals to services and health care including but not limited to medication for opioid use disorder, drug treatment, housing, mental healthcare and medical care. The Health Program Coordinator will provide support for presenting, and training community groups, the NYPD, and other law enforcement partners on PORT, naloxone, drug trends, and harm reduction techniques; and conduct drug checking. The incumbent will work with appropriate staff to contribute toward identifying and recruiting new sites for drug checking. In addition, the incumbent will aid in promoting drug checking within the communities where the service has already been established; and perform other appropriate related duties.

Minimum Qualifications

Bachelor's degree in a related field and two years of experience contributing to the coordination of program activities in a public health, health, health regulatory, or human services related program; OR an Associate's degree in a related field and four years of such experience; OR six years of such experience. A Master's degree in a related field may substitute for one year of experience.

Preferred Qualifications

Work experience providing direct services to individuals with a history of substance use using a harm reduction framework. Demonstrated, work-based understanding of illicit drug use, overdose, HIV, viral hepatitis, trauma, and stigma as experienced by people who use drugs. Work experience with conducting post overdose interventions in home settings as well as street- based outreach. Thorough understanding of the informed consent process. Experience counseling persons regarding overdose prevention planning. Bilingual in English and Spanish.

Conditions of Employment

Grant funded position. Compliance with funding requirements such as time and effort reporting, grant deliverables, and contract deliverables, is required.

Valid and unrestricted authorization to work in the U.S. is required. Visa sponsorship is not available for this position.

Travel up to 90% of the time within the five boroughs of New York City will be required. A valid driver's license in good standing is required for travel to areas not served by public transportation.

The incumbent in this position may be required to work evening and weekend hours.

The selected candidate must reside within a reasonable commuting distance of the official work location specified in the job posting and must also be located in or willing to relocate to one of the following states: New York, New Jersey, Connecticut, Vermont, or Massachusetts, prior to hire. Telecommuting will be available.
