The Community Resource Specialist role is essential to provide UnityPoint Health Meriter medically homed patients with comprehensive care and work to address social determinants of health. This position collaborates with other clinical staff to assess and triage patient social service and community resource needs and coordinate services and referrals as appropriate. The primary role of the Community Resource Specialist is to link community resources to patients in need.

Required Qualifications:
  • High school diploma or GED
  • 3 years of experience in community agency providing direct care services to clients.
  • Valid driver's license when driving any vehicle for work-related reasons.
  • Microsoft software skills with programs such as PowerPoint, Excel, Word, and Access.

Preferred Qualifications:
  • 4-year degree in health care, social services, or related field
  • Experience in case management
  • Experience with diverse cultures
  • Knowledge of medical software applications (ie. EPIC)
