Foothill Family's Nurturing Connections Program is designed to establish sustainable, integrated network of care that bridges behavioral health, medical, developmental, and nutritional support services for birthing people, children ages 0-5, and their caregivers. The Quality Improvement (QI) Specialist is responsible for monitoring program fidelity, tracking performance indicators, and driving continuous quality improvement for the Nurturing Connections Program. This role ensures that services are delivered in alignment with established protocols, screening tools are administered as required, and documentation in the EHRS meets accurate and compliance standards.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Supports and promotes the mission of the Agency: Foothill Family empowers children and families on their journey to achieve personal success.
  • Analyze program data, identify trends, and work with Nurturing Connections leadership and collaborative partners to develop solutions that enhance service effectiveness.
  • Will support ongoing training, to ensure programming goals are met and quality benchmarks are consistently achieved.
  • Manages data validation processes and refines data management procedures.
  • Provides training to Family Support Specialists and ensures the accuracy and quality of reports that are submitted.
  • Review services delivery workflows and offer real-time implementation insight to team members.
  • Identifies trends and patterns to measure Initiative's ability to meet community needs.
  • Locates and defines new process improvement opportunities and recommends upgrades or improved reports for better decision-making tools.
  • Prepares reports on program goals and objectives and identified performance metrics.
  • Follows all policies, procedures and legal mandates including HIPAA, client confidentiality, releases of information, protected health information (PHI) and client files.
  • Provides services relating to the administration of public funds or benefits, including eligibility for public funds or public benefits.
  • Travels between Foothill Family sites for training, meetings, and other in-person interactions.
  • Attends in-person meetings and events at various locations within the Los Angeles County and surrounding areas.
  • Displays sensitivity to the service population's cultural and socioeconomics characteristics.
  • Performs work in a safe manner at all times and ensures that other individuals do not put themselves or others at risk.
  • Follows all policies, procedures and legal mandates including HIPAA, client confidentiality, releases of information, protected health information (PHI) and client files.
  • Ensures performance and behaviors support the Agency's performance quality improvement (PQI) goals and objectives.
  • Reports to work on time and maintains reliable and regular attendance.
  • Performs other duties as assigned.

POSITION REQUIREMENTS

  • Bachelor's degree in health education, social sciences, public health, education or related field preferred.
  • Knowledge and experience in administration, data analytics, and utilization of electronic reporting and record-keeping systems.
  • Ability to administer a complex program by being detail oriented, understanding the systems which help ensure quality of service and accurate record keeping.
  • Ability to communicate detailed, technical information, both orally and in writing.
  • Local travel between office, school, and community sites required.
  • Valid CA Driver's License and maintains insurability on the Agency's auto liability policy (including a minimum of 2 years driving experience) and maintains the California state required auto insurance liability limits OR maintains reliable transportation to attend all appointments and meetings as required in the position.
