Position: Complete Health Nurse (RN or LPN) for Population Health RN Salary Wage Range: $87,000-$90,000 annually, depending on experience LPN Hourly Wage Range: $26.00 - 32.00 per hour, depending on experience Service Area: Northwest Metro (coverage area: Cokato, Winsted, Excelsior, Buffalo, Champlin, Loretto, Anoka, Osseo, Rogers, Elk River, Rogers, Ramsey, Maple Grove, Zimmerman, Albertville, Bethel & surrounding). Plus, mileage reimbursement of .70 cents per mile. Schedule: 1.0 FTE, Monday - Friday, 8:00am-5:00pm (no holiday, on-call or weekend rotation) Lifespark Benefits Include:
The Complete Health Nurse is a key member of the Lifespark COMPLETE team that serves Seniors in their homes. The Complete Health Nurse serves as a nursing expert who plays a critical role in onboarding new members to the program, supports medical needs throughout their time on service, and supports members during vulnerable periods of great need. Complete Health Nurses support the delivery of expert geriatric primary care by taking a whole-person approach to delivering home-based medical services. They will work collaboratively with a broader interdisciplinary team of providers and other allied health professionals to improve their members' health.
