SUMMARY We are currently seeking a Social Worker II to join our team in the Redleaf Center. This part-time role will work 40 hours within a two week pay period (on-site). Group clinic is on Fridays (required) with flexibility on the other scheduled days. This role is currently grant funded through the end of 2027. Purpose of this Position: Provides direct service to clients and their families at the Redleaf Center for Family Healing within Hennepin Healthcare. This role will work directly with patients who are pregnant or have children under the age of 5 and have mental health needs and current or history of substance use. This includes social work assessment, interventions and treatment plans, supportive counseling and crisis intervention, care management, advocacy and support. This role will also heavily entail seeking resources and services to address unmet needs, but also collaborating with community programming including Child Protection. The social worker also provides consultation services and in-service education programs related to the psychosocial aspects of health care to other professional staff members on the healthcare team.
