OverviewThe Pharmacist is responsible for preparing and dispensing of medications (including sterile admixtures and non-admixtures), and all related equipment and supplies in accordance with physician orders, URAC and ACHC standards, federal and state laws and regulations, and Reliance Rx policies and procedures. Must ensure that all required documentation is complete and that all products meet stability, compatibility and practice standards. Must evaluate physician ordered medications for appropriateness of therapy, and maintenance of patient specific optimal therapy through clinical management of medications provided in relation to individual diagnosis. Clinical responsibilities include but are not limited to preparation of patient records, maintaining a medication profile, care planning, evaluation of drug interactions, preparing progress/communication notes, clinical monitoring, pharmacokinetic monitoring, renal dosing, laboratory and culture monitoring, determining medication administration compliance and effectiveness, and discharge planning. Must provide drug information, perform clinical consultation and patient counseling, and communicate with physicians, nurses and patients. Must also perform ordering/management of medications, controlled substances, equipment and supplies as necessary or assigned. Professional working relationships with pharmacy staff and others must be maintained in order to coordinate clinical team activity appropriate to providing patient care. Qualifications

In addition to base compensation, associates may be eligible for a scorecard incentive, full range of benefits and generous paid time off. The base salary range is subject to change and may be modified in the future.
