Overview

We are seeking an Inpatient Pharmacist, also referred to as an Acute Care Pharmacist or Hospital Pharmacist. As a Hospital Pharmacist for our Inpatient Pharmacy, you will prepare, dispense, and manages medications for patients in our hospital under the supervision of our Pharmacy Director. Hospital Pharmacists work closely with doctors and nurses to ensure medications are safe, effective, and correctly dosed, while also monitoring for side effects or drug interactions. This position is on a per diem basis, meaning PD/PRN as needed.

** Approximately one weekend every five weeks, rotating through main pharmacy; clinical monitoring on the floors on the day shift.



Responsibilities

Essential Duties and Responsibilities (includes, but not limited to):

  • Conduct patient interviews to obtain accurate and complete medication histories during admission or transition of care
  • Review and verify physician medication orders for appropriateness, dosage, route of administration, frequency, and potential drug interactions
  • Ensure medication orders comply with hospital protocols, clinical guidelines, and patient-specific factors (e.g., renal function, allergies, weight)
  • Prepare and dispense medications accurately, including IV admixtures, unit-dose medications, and controlled substances, following all safety and sterility protocols
  • Monitor medication therapies and provide clinical recommendations to optimize therapeutic outcomes and minimize adverse drug events
  • Collaborate with physicians, nurses, and other healthcare professionals to develop and adjust patient-specific medication plans
  • Coordinate daily operations within the pharmacy to ensure efficient medication distribution and inventory management
  • Deliver pharmaceutical care to a wide range of inpatient units, including - General Medicine/Surgery, Intensive Care Units (ICU), Obstetrics & Gynecology, Pediatrics, Emergency Department, Oncology, Perioperative Services, Geriatrics and other departments


Qualifications

Before we go any further, we do have some deal-breakers. You must have:

1. Current state Registered Pharmacist License.

2. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.

Additional Qualifications That Are a Plus:

1. Experience in hospital pharmacy operations preferred.

2. BLS certification preferred
