Overview

CJE SeniorLife Supportive Services create a caring environment dedicated to helping older adults stay independent and connected. Whether it's securing legal or financial assistance, making connections with new friends, or getting support through challenging times, we match older adults with the right program to address their immediate and long-term needs. Services we provide include benefits services, care management, caregiver resources, counseling services, food services, Holocaust Community services, and legal services. We are part of the Jewish Federation of Metropolitan Chicago.

The Lakeview Program Coordinator oversees a new initiative to increase outreach and services to older adults and their family caregivers in the Lakeview community on Chicago's north side. This role combines community outreach, care management with an emphasis on linkage to CJE SeniorLife and other aging services, and occasional public speaking to both community members and professionals.

Compensation: $70,000 USD per year

Shift: 8:30am-5:00pm Monday-Friday, occasional evening or weekend hours for a speaking engagement.

Work Location: This is a full-time, in-office role at our Rogers Park office, Lakeview synagogues, and field visits

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.

Clinical & Direct Support Services



  • Identify families in need of social services and connect them to appropriate home- and community-based services including those offered by CJE SeniorLife.
  • Directly provide care management services where needed for Lakeview area residents.
  • Conduct comprehensive assessments and develop personalized care plans.
  • Maintain accurate and thorough documentation of client work.
  • Develop and deliver educational programming for older adults and family caregivers on topics such as accessing resources, aging well, and navigating care systems.
  • Serve as a subject matter expert on aging resources in the Lakeview community.
  • Lead or co-lead caregiver support groups and other group programming to address needs identified in community assessments.
  • Collaborate on designing innovative older adult and caregiver support programming that goes beyond traditional models.


Program Development & Community Partnerships



  • Identify organizational partners in the Lakeview area including local synagogues.
  • Develop working relationships with community partners to further program goals.
  • Represent CJE SeniorLife at community events and stakeholder meetings.
  • Work in tandem with CJE's Community Outreach staff to plan and implement programming.
  • Document activities related to community outreach.
  • Track program metrics and outcomes to evaluate effectiveness.
  • Create and conduct professional development sessions for social service providers, healthcare professionals, and synagogue partners on aging-related topics and available resources.
  • Collaborate with CJE Marketing team to develop targeted outreach materials for both community members and professionals.
  • Work collaboratively with synagogue leadership while maintaining CJE's professional standards and practices.
  • Other duties as assigned.



Qualifications

Education and/or Experience



  • Master's degree in social work or related field strongly preferred.
  • 3+ years of experience in social services, care management, community outreach and related roles.
  • Experience facilitating support groups and designing innovative group programming.
  • Preferred: Professional certification in care management or case management such as CMC, CCM, C-ASWCM, or C-SWCM. Candidates without certification must be willing to pursue appropriate certification within the first two years of employment. CJE SeniorLife supports this professional development through paid time for training and certification costs.
  • Familiarity with community resources and social service environments, especially those serving older adults.
  • Proficiency with Microsoft 365 suite and client management systems (Salesforce experience preferred).
  • Exceptional organizational skills and attention to detail.
  • Proven capacity to develop relationships with external partners.
  • Outstanding written and verbal communication abilities.
  • Capacity to work with a high degree of autonomy as well as collaboratively.
  • Ability to navigate complex stakeholder relationships while maintaining professional boundaries.


Language Skills



  • Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.


Other Qualifications



  • Commitment to person-centered trauma informed principles.
  • Excellent clinical and interpersonal skills.
  • Ability to maintain strict confidentiality with sensitive information.
  • Flexibility and willingness to help develop and implement a new role at CJE.
  • Some local travel is required.
  • Familiarity with the Jewish community and Jewish cultural practices strongly preferred.
