Job Description

JOB SUMMARY

The Care Coordination Specialist serves as an advocate and single point of contact for assigned patients and families coming to Stanford Children's Health for destination services. The Care Coordination Specialist is responsible for assisting patients and families pre-admission with all non-clinical aspects of the anticipated care at Stanford Children's Health including transportation, equipment, housing, and other required resources. Evaluates and identifies barriers to medical treatment, ensure access to appropriate resources, improve patient experience, and resolve issues that could impact care progression. Serves as the liaison between the clinical staff and family by orienting and educating related to treatment plan, coordinating referrals, and ensuring all resources for appropriate care are in place. Generates new resources and referral options for patients and families based on needs.

ESSENTIAL FUNCTIONS

Must perform all duties and responsibilities in accordance with the hospital's policies and procedures, including its Service Standards and its Code of Conduct.

* Acts as non-clinical liaison for patients coming to Stanford Children's Health for destination services during pre-admission, assuring care continuity by acting as a single point of contact. Works closely with transfer center, authorization, financial counseling, scheduling, and clinical teams, including the referring interdisciplinary team, to ensure that non-clinical needs are met.
* Coordinates transportation, equipment, housing, and other required resources for a smooth care progression.
* Orients and educates patients and families, in conjunction with other care providers, related to treatments, procedures, medications, and continuing care requirements.
* Coordinates with patient financial services to ensure authorizations and letters of agreement are in place for patients coming to Stanford Children's Health for destination services. Follows up post-discharge to overturn denials. Works with insurers to troubleshoot and supply any data as needed.
* Conducts assessments on patient finance and authorization to make recommendations on appropriate referrals or other resources.
* Communicates utilization review, discharge planning, psychosocial considerations, financial needs, and other related information to appropriate care providers and community agencies.
* Maintains database related to discharge planning, utilization review, housing, transportation, patient care funds, community referrals, and quality indicators in the medical record. Provides reports as needed.
* Evaluates outcomes of care with interdisciplinary team to decrease hospital readmissions. Based on results, develops and implements of new programs, operating protocols, and communication initiatives related to care coordination. Participates in goal setting and process improvement to advance desired care team outcomes.
* Forwards admission, concurrent and retrospective reviews under the direction of the Case Manager, reviews daily admission data and checks inpatient insurance benefits, authorized days and other related needs.
* Availability by phone required for after-hours coordination needs for designated patient population.

Qualifications

MINIMUM QUALIFICATIONS

Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.

Education Required: Bachelor's degree in a related field

Experience: 3 Years of directly related experience

Licensure/Certification: None required

KNOWLEDGE

* Knowledge of computer systems and software used in functional area.

Pay Range
Compensation is based on the level and requirements of the role.
Salary within our ranges may also be determined by your education, experience, knowledge, skills, location, and abilities, as required by the role, as well as internal equity and alignment with market data.
Typically, new team members join at the minimum to mid salary range.
Minimum to Midpoint Range (Hourly): $35.48 to $47.07
