Position Summary: The Outreach Services Associate is responsible for conducting targeted community outreach and engagement, risk reduction education, referrals, and case documentation including appropriate follow-ups based on acuity levels. This role involves working with vulnerable communities such as LGBTQ+, homeless, substance users, and others. The associate will drive the mobile van to various locations within Queens, Nassau, and Suffolk.
Key Responsibilities:
- Conduct targeted community outreach and engagement, providing risk reduction education, referrals, and case documentation with appropriate follow-ups based on acuity levels.
- Conduct comprehensive assessments of clients' needs for primary care treatment and/or supportive services, including early interventions such as PrEP/PEP, peer-based counseling, and health education tailored to clients' health literacy levels and language preferences.
- Provide up-to-date HCV, Syphilis and HIV-related risk reduction sessions on-site and off-site, referring clients based on their behavioral/sexual risk assessment/profile.
- Secure and maintain health screening exams in a HIPAA-compliant environment.
- Identify new community partners and data-driven hotspots for conducting HIV / Syphilis and HCV rapid testing.
- Complete required data collection.
- Assist clients in navigating healthcare services systems.
- Coordinate care with Linkage to Medical Case Managers.
Additional Responsibilities:
- Drive the mobile van to various locations within Queens, Nassau, and Suffolk.
- Perform other duties as assigned.
Job Qualification
- High School Diploma or equivalent required.
- 0-1 years of relevant experience, required.
- Bilingual, Spanish speaking highly preferred.
- Previous Outreach experience
- Previous experience working with the LGBTQ+ community.
