Responsibilities
  • Provide In-Home Speech Therapy - Conduct initial and comprehensive assessments, develop individualized treatment plans, and deliver speech therapy interventions to improve language, swallowing, cognition, and memory deficits.

  • Collaborate on Patient Care Plans - Work with physicians and interdisciplinary teams to develop and implement home health care plans that align with best practices and ensure high-quality patient outcomes.

  • Ensure Safe & Effective Therapy - Utilize appropriate treatment modalities, communication devices, and therapy techniques while maintaining patient safety and delivering exceptional care in accordance with agency policies.


Qualifications
  • Master's degree in speech-language pathology Licensed as a speech-language pathologist by the state(s) where providing services
  • If state does not require SLP license - completion of 350 clock hours of supervised clinical practicum; completions of 9 months of supervised full-time speech-language pathology services after obtaining a master's or doctoral degree; and completion of a national examination in speech-language pathology approved by the Secretary of Health and Human Services.
  • One year experience caring for similar patient population as to be assigned.
