As a successful candidate, you will:

The Clinical Informatics Pharmacist acts as a liaison between the clinical departments and IT. Leveraging both clinical knowledge and informatics knowledge, the Clinical Informatics Pharmacist provides innovative solutions utilizing best practices and use of standards to help facilitate the creation of scalable and maintainable system solutions. The Clinical Informatics Pharmacist will work with multidisciplinary teams including physicians, nurses, pharmacists and other health professionals as well as operational leadership and IT to promote safe, efficient, effective, timely, and optimal use of medications. The Clinical Informatics Pharmacist, through multidisciplinary collaboration ensures that system design follows best practice design principles. Will develop clinical content related to medications in collaboration with other key clinical stakeholders for clinical applications such as Epic and will coordinate activities related to Clinical Pathways. Works closely with the build team to ensure product functional design matches system capabilities and architecture.

  • Provide leadership and expert consultation on the design, development, and implementation of process and programs related to the clinical information systems and Electronic Health Record pertaining to medication management and in alignment with The Joint Commission standards
  • Facilitate multidisciplinary team collaboration- physicians, nurses, pharmacists and other health care professionals to evaluate current state and develop future state design of medication management review and updates.
  • Facilitate timely review and updates oof order sets and treatment plans as required by City of Hope policy and escalate concerns of non-compliance to the physician quality, nursing and pharmacy leadership if/when needed
  • Coordinate teams and direct members for project planning and execution.
  • Assist in designing the system that is user-friendly and that the User Interface layer presents content, format and navigation in a logical, useful and efficient manner for clinical use adhering to procedures and policies of the organization.
  • Develop content and processes with the Pharmacy system in mind and strive for standardization
  • Coordinate Clinical Pathway activities
  • In collaboration with key stakeholders, assess clinician enhancement requests, and champion the requests through the Governance Committees and change management processes.
  • Assist IT when necessary to troubleshoot and work with the appropriate teams to develop resolution for issues that are identified.
  • Utilizes project management methodologies when appropriate.
  • Ensure that design connects evidence-based medicine literature and guidelines and promotes evidence-based practice.
  • Maintain a process for periodic review of application content with updates to content as required by evidence and best practices and clinical requirements.
  • Ensure that design reduces unnecessary variation in care and care processes promoting standardization across COH System.
  • Understand application requirements from pharmacy, clinical, quality, regulatory, and operational perspectives.
  • Create and implement change management plans through a predesigned framework and model gleaned from best practice when appropriate
  • Learn industry trends and recommend improvements of application, training and adoption.
  • Responsible for facilitating and providing department specific training or conduct training when needed.
  • Position may require travel between various community clinics and its affiliates.
  • Performs other related duties as assigned or requested.

Qualifications

Your qualifications should include:

  • Degree in Pharmacy (Pharm D)
  • 5+ years in a pharmacist role or
  • 3 years in a Pharmacist role and 2 years of experience in healthcare Information Technology experience.
  • Active board of pharmacy license
