OUR TEAM AND DEPARTMENT
Our department includes nurse navigators, social workers, dietitians, chaplains and music therapy. We are located in all 4 oncology clinics for Simmons in Ft Worth, Richardson, Dallas, and Redbird.
Our Dallas social work team is assigned to diagnosis-specific clinics along with providing team coverage when needed.
This social work position will support the clinic and infusion areas by responding to walk in requests, clinical referrals, and patient distress screeners.
This is a full-time hybrid position after a 90 day on site onboarding process and approval sign off from our leadership. Typically, our social workers are at the Dallas clinic 3 days per week, and working remotely 2 days per week.

EXPERIENCE AND EDUCATION
Required
- Education
Master's Degree in social work.
Preferred
- Experience
Experience in hospice and/or neurology conditions. LCSW certification.
JOB DUTIES
- Assists in development and implementation of Case Management. Provides expertise in area of social work as it pertains to management of patient.
- Assists in coordination of client care through formulation of appropriate plan of care via accurate assessment of patient's social, financial and insurance situation.
- Develops short- and long-term plans to facilitate patient care and discharge.
- Utilizes crisis intervention and problem-solving skills to facilitate and enhance patient and family's ability to meet acute and ongoing medical and psychosocial needs.
- Networks with community agencies to assure appropriate client access.
- Maintains documentation and meets department and University Hospital documentation standards.
- Identifies cases with ethical and legal considerations and makes referrals. Confers with supervisor.
- Communicates with other health care providers, patients, families, other hospital personnel, and visitors.
- Demonstrates effective communication with adolescents, adults, and older adults.
- Maintains system for tracking placement of patients after discharge on monthly basis.
- Has knowledge of Department and Unit Performance Improvement program and is aware of expected participation. Identifies need for professional growth and seeks development opportunities.
- Provides education to staff, patients, and community resource affiliates.
- Performs other duties as assigned.
