Position: Quality Improvement Process Specialist

Job Description/Requirements:

As a Quality Improvement Process Specialist you will:

In conjunction with Agency management, develop and implement tracking and/or monitoring systems to ensure the Agency's compliance with federal, state and/or county regulations and accreditation standards.

  • Evaluate tracking / monitoring systems for compliance to agency processes and vendor requirements by performing reviews that includes but are not limited to EVV and verification audits, operations reporting, plan of care and task reconciliation.
  • Recommend process changes to enhance operations or improve outcomes as needed.
  • Prepare and submit compliance reports to the Agency's QAPI Committee.
  • Aggregate, analyze and report data on a variety of Agency specific topics in verbal and/or written

  • Assist in the review and analysis of financial, clinical, and operational performance indicators as needed.
  • Participate as required in process improvement and training initiatives.
  • Guide branch staff in developing quality improvement data and collection procedures.
  • Establish and maintain effective and cooperative working relationships with management and staff at each branch site on compliance activities/requirements, etc.
  • Assist branches with credentialing and contract maintenance.
  • Comply with applicable legal requirements, state regulations, accreditation standards, policies and
  • Procedures including but not limited to the Corporate Compliance Program, Code of Conduct, HIPAA, and regulatory-specific documentation standards.
  • Conduct on-site audits at each licensed branch office for process compliance with agency processes and state regulatory requirements and accreditation standards by reviewing personnel files, policies and procedures, incident logs, emergency preparedness, infection control.
  • Comply with applicable legal requirements, standards, policies and procedures including, but not limited to the Compliance Program: Code of Conduct, HIPAA and Documentation Standards.

Qualifications

Requirements of Qualified Candidates:

Minimum of 3 years of related work experience with a concentration on quality improvement and/or risk management programs, data gathering, analysis and reporting

Bachelor's degree in Health, Human Services, or Business preferred

Computer proficiency and significant working knowledge with Microsoft Word, Excel and Power Point programs is required

Knowledge of state regulations and accreditation standards for home care preferred

Strong time management, prioritization and organizational skills

Strong written and verbal communication skills

Ability to work in a fast paced, multi-task environment with competing priorities

Willingness to take on new projects and initiatives

Excellent interpersonal/listening skills; able to interact and create a collaborative working environment among internal and external stakeholders

Excellent problem solving and task analysis skills

Possess a responsible, detail-oriented, results-driven work ethic
