The UNM Medical Group, Inc. is hiring for a Clinical Nurse Coordinator to join our Office of Population Health Management department. This opportunity is a full-time day shift opening located in Albuquerque, New Mexico. This is a hybrid position. The ideal candidate will need to be willing to perform Annual Wellness Visits for multiple ambulatory clinics, provide virtual visits, and work remotely. Minimum $75,670 - Midpoint $96,439* *Salary is determined based on years of total relevant experience. *Salary is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE. Summary: Under general supervision, provides clinical and operational support to healthcare providers and patients. Serves as the liaison between the healthcare team and the clinic, acts as the primary contact for referring providers and patients, and works closely with healthcare professionals to ensure patients are appropriately scheduled for necessary tests and procedures. Addresses patient concerns and facilitates smooth coordination of services across multiple departments, clinics, and healthcare facilities. This role also serves as a resource and educator for both patients and staff. Patient care assignments may include individuals of all age groups. Minimum Job Requirements of a Clinical Nurse Coordinator: Associate degree in nursing with 2 years of nursing experience. Licensed Registered Nurse (RN) in State of New Mexico. Completed degree from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis. Verification of education and licensure will be required if selected for hire.
