Description

JOB PURPOSE:

Ensures delivery of services to families and youth by linking them to crisis pregnancy services, foster care services, and or other community services. This program services 9 Texas counties: Austin, Brazoria, Chambers, Fort Bend, Galveston, Harris, Liberty, Montgomery, and Waller counties. This position supports rural geographical regions and travel is involved. This is a grant-funded program.


Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Develops effective, trusting relationships with program participants by utilizing trauma-informed care and a strength-based approach. Models positive interactions, verbal guidance, and effective communication.
  • Develops and maintains comprehensive outreach/working relationships with Clients and community partners on the participants' behalf to develop workable plans to obtain and/or enroll in community services.
  • Reviews client's needs assessment, connects clients to services in the community, and tracks outcomes.
  • Follows care follow-up procedures to ensure families receive the services they were referred to.
  • Assists in conducting outreach and networking activities in the community. Provides presentations at various community groups/events. Is active and networks in monthly community meetings. Promotes the success and reputation of the Houston FYSS program.
  • Other duties as assigned.
  • Demonstrate Exceptional customer service, in Everything you do, by placing the child, family, Veteran or client first to support our mission to "Empower people to build better lives for themselves, their families, and their communities."


ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's Degree

EXPERIENCE: 1+ years of experience in a social service agency, 1+ years in a customer service-focused environment. Experience with mental health and/or a homeless population is a plus.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Bilingual Spanish/English preferred. Must be available and willing to travel to various locations and at the frequency that the business needs dictate.
