Description MaineHealth Home Health and Hospice ~ 32 hours/week, Full-time Days~ Maine state licensure to practice as a Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker - Clinical Conditional (LMSW-CC) is preferred or Licensed Social Worker (LSW)is required Home Care Position Highlights:
Position Summary The Home Care Social Worker supports MaineHealth Home Health and Hospice's mission, vision, and values by exhibiting the following behaviors: excellence, competence, collaboration, innovation, respect, patient and family centered care, commitment to our community, and accountability. The Social Workerprovides skilled, cost effective social work care in a competent holistic manner for a designated group of patients, through assessment, planning, implementation and evaluation of social work care, to empower patients and families to achieve maximum levels of wellness and independence. Required Minimum Knowledge, Skills, and Abilities (KSAs) 1. Education: Bachelor's Degree in Social Work required. Master's Degree in Social Work preferred. 2. License/Certifications: State of Maine licensure as an LMSW or LCSW required. Valid Driver's License, with vehicle available for work covered by liability insurance as specified by agency policy. Current BLS Certification required or must obtain within 30 days of start date. 3. Experience: LCSW and LSW's require two years of experience and ongoing supervision as required by state required. LMSW's requires one year of professional experience. 4. Additional Skills/Requirements Required: N/A 5.
